I have multiple companions in Act 1 that have contracted Filth Fever. After 2 consecutive saves, it should go away. Lann has succeeded on it more than 6 times now in a row, and he still has it. The problem is that most diseases require a minimum of two consecutive saves. So for filth fever you PC could fail the first save and have an onset of 2 days, then make the next save and still be sick. Here is a brief example of how this could play out.
